This is the process followed for claim settlements |

|
- Intimation
The family of the deceased Life Assured intimates the company about the death of the life assured through a Branch executive /Sales officer by giving a written intimation & a copy of the death certificate issued by the Municipal Authority/ Gram Panchayat (mandatory requirement for logging a claim) to the Branch. The Claims Department at BSLI, Head Office can also be directly intimated about the claim.

- Sending a notification
Once the Claims Department receives the intimation of a claim, a notification letter is sent to the claimant along with all required claim forms, depending on the type of the claim and the branch which has been intimated of the same.

Normal Requirements in Case of a Death Claim: |
|
|
- Copy of Death certificate duly attested by the authorised person in the Branch
|
|
- Original Policy document / Indemnity Bond in case of Loss of Policy Document
|
|
|
|
- Legal requirements (in case where title to policy monies is not clear)
|
|
- Death Abroad Questionnaire (if death has taken place outside India in a foreign country)
|
|
- Medical Attendant’s Certificate
|
|
- Employer’s Certificate, if applicable
|
|
- Copies of Medical Reports of last and previous hospitalizations, if any
|
|
- Any other requirement as warranted by the facts of the Claim
|
|
|
Accidental Death Claim with or without Rider (In addition to the above requirements) |
|
All copies duly attested by the authorised person in the Branch |
|
|
|
|
|
|
|
|
- Newspaper Cuttings, if any
|
|
|
Note In case the claim warrants any additional requirement, BSLI reserves the right to call for the same. Also submission of the claim requirements does not mean admission of the claim liability by the Company. |

- Certificate of treatment in hospital
|
|
- Examination by our appointed physician, if required
|
|
|
Critical Illness Claim |
|
|
|
|
|
|
- Certificate of treatment in hospital

|
- Family Physician’s Certificate
|
|
- Attested Copies of all other Medical Reports
|
|
- Examination by our appointed physician, if required
|
|
|
The claim has to be lodged by the Life Assured after expiry of a period of 30 days from the date of diagnosis. |
